How We Started
A Shared Vision
Navigate was founded through a shared vision of a geriatric care manager and a Hospice and ICU nurse. Working in the system, they realized how fractured and broken it was and how it was failing our aging loved ones.

My lifelong love for older adults has led to professional experiences in mental health case management, low-income housing, and long term facility care coordination (skilled nursing rehab to home, long term care, and admissions).
During all these experiences I saw a recurring theme — the huge gap between the silos of healthcare and community. I had questions. Who was helping people in the community when they left? Who is taking the time to explain changes? Who is checking on the caregiver? Is anyone helping people prevent these problems?
So, in 2018 I started a Geriatric Care Management company to become part of the solution, to become a partner to those drowning in the overwhelming world of aging care. After owning & operating that company for several years I knew it was time to grow to more than just me. I believe people get the highest quality care when medically-minded & holistic-minded people work as a team. This is exactly what we are passionate about at Navigate Wellness – heart-centered, human-forward, & well rounded care navigation that keeps the older adult in the drivers seat. There is empowerment that happens when a person can be met exactly where they are & access personalized support, education, and advocacy.
