Questions About Navigate Wellness
Is my mom or dad safe at home?
This is one of the most common questions we get, and unfortunately, there is no simple answer. Each individual is different and has their own unique needs.
All our services start with an Aging Road Map, a detailed assessment of your loved one’s health needs, environment and other factors. Through this process, our team can assess the health of you or your aging loved one and help determine how to keep you safe at home longer – or help you decide it’s time for other living arrangements.
Do you accept insurance?
No, we do not accept insurance and are private pay only. We aim to keep our services affordable to help as many people as possible. With our personalized care plans, you’ll get exactly the support you need – and nothing that you don’t. We know that the longer you or your loved ones can stay in your home, the more affordable your healthcare will be.
We don’t take insurance but we will help you utilize what insurance provides and fill in the gaps of what they don’t cover. There is no ability to tailor care when you work with insurance. It is one size fits all which is the opposite of what we do. Without insurance involved we are able to give you more choices and freedom.
Our care management services include helping put your insurance to work for you, and negotiating with insurance companies on your behalf. We speak insurance and have a background in working with them and advocating for you.
What kind of health challenges do you help with?
Our team of skilled concierge nurses in Columbia, MO have years of experience in ICU and ER settings, so they’re trained to handle just about any kind of health issue or emergency. Some of the most common things we help our patients with include: wound care, post-op recovery, medication management, heart health support, blood pressure monitoring, IV infusions, diabetes management, mobility support and more.
Our care management team pulls all of these services together, ensuring a coordinated effort between Navigate Wellness and all of your providers. Clear communication and collaboration ensure you get the best care possible.
What type of care is provided during nursing care visits?
There are so many ways that Navigate Wellness can help. We can also help clients with showering, reviewing any new or ongoing symptoms and helping them make additional appointments with their providers.
We also ensure your loved one is getting regular check-ins from a trained medical professional, so we can proactively catch any signs of new decline. Our team can coordinate our services with your loved one’s existing healthcare team, including any doctors and specialists.
What if my loved one needs to go to the hospital?
If you or your loved one needs to go to the hospital, our nursing team can help. In-person or over-the-phone support can help you determine if a hospital visit is needed. If you do go to the ER, we can meet you there to provide support and ensure you have a trusted professional with all of your medical information on hand.
We aim to make any trip to the hospital easier. Before you leave home, we’ll tell you what to expect, and what tests should be run and help prepare you to answer questions. Our goal is to create truly informed consent, reduce the overwhelm of appointments, and get you home quicker.
Because our team members have worked inside the hospital system, we know what to expect and share that knowledge with you.
Do you have minimum monthly hour requirements?
Plan requirements depend on your specific package. On average, our clients are seen twice a week. Eight total monthly visits is ideal. The types of visits can vary based on your needs, with a mix of nursing visits and care navigation appointments.
