How we support our clients leaving the hospital

One of our clients went into the hospital for an outpatient procedure. We were aware this was happening and offered to come alongside them as part of our Hospital to Home service. This means we were fully prepared to show up to the hospital with our patient to help them through the discharge process, answer any questions, and be a sense of support and guidance during this time. 

The client declined having that level of support and opted for his daughter to take him, which we respected. 

The following day, our client had a complication with the procedure. We found out that the discharge instructions given to our client weren’t very clear. And our client was concerned about how he was progressing post discharge. 

Our client and his family reached out to us and we visited his house to reassure the family that he is progressing well. 

All of this worry and back and forth could have been prevented if our client had allowed us to be there during discharge, hear the instructions, and ask the doctor all the questions that the family might not know to ask, or maybe are too intimidated to ask. This is exactly why we developed our Hospital to Home package. To help clients navigate not only hospital stays but the often overwhelming discharge process and those first few days back home on your own.
