Nurse Navigator Job Description

• Travel to client homes and manage care plans according to physician instructions. 
• Develop and implement comprehensive nursing care plans tailored to each client’s specific needs.
• Provide guidance and oversight to companion caregivers, ensuring they follow the established care plans and delivery quality support.  
• Collaborate with an interdisciplinary team of health professionals to address all aspects of patient care. 
• Monitor and report changes in patient condition. 
• Administering or filling medications according to prescribed schedules to ensure accurate and timely administration for patients. 
• Inspect wounds, change dressings, and assist in personal hygiene.
• Maintain accurate patient reports in accordance with healthcare regulations and organizational policies. 
• Educating patients, caregivers, and family on the aftercare or ongoing care of the patient. 
• Perform other duties as assigned.

Job Qualifications

• RN license valid in the state of Missouri.
• Valid drivers license and reliable transportation.
• Ability to do extensive bending, lifting, and standing on a regular basis. 
• Ability to pass an FSCR background check.
